Sir/Madam:
I would like to request for an update of my PERAA benefit. I applied for an early retirement last May 2009 and up to now I have not receive my emplyees’share.The last thing I’ve heard was that there was a mistake on the form that I signed so I have to sign another in which I did.When I made a follow up in your office through phone, they said that some documents are still lacking.So I have it checked from my employer (University of Pangasinan).They said that you have not sent yet the needed documents to be signed by us.
How many more months should I wait before I could receive my benefit?And how many more days should I wait before I could sign the lacking document which your office will send to my employer? I might leave the country so soon and I might as well not sign it.My family is needing the said benefit for them to use and besides we will need that to pay the debt I incurred in the processing of my visa.I do hope you understand my situation.
I look forward for your immediate response on this matter.
